I think the fan switch is up with the thermostat body, in the pipework above the rocker cover (below the fuel tank). A shortage of water could mean the fan switch isn't being bathed with coolant. Eventually it probably should still kick in because of the heat from the steam, but I think that direct contact with liquid coolant still works better.

The water for the cold start mechanism is plumbed into the lower of the two radiator spigots you already identiified.
